Understanding the needs of chronically ill individuals and their family, we offer comprehensive annual membership plan. We guide patients to best plan for them based on their needs and our initial complimentary evaluation.

Annual Membership Tiers:

Compassionate Care Membership for Health Challenges
(all price in USD)

One-time Assessment:

Comprehensive Assessment $385 one-time assessment (no membership required)

Each client is unique. Our assessment includes review of client’s case and detail the needs which can include medical, non-medical and support services and how we can assist with a detailed evaluation and plan.

Basic Membership:

Annual Fee

$495

(6 hours of care coordination included, additional hours at hourly rate of $175)

Services Include:

  • Month 1 and Month 6 assessments and personalized care plan development
  • Comprehensive Insurance review
  • We access most cost-effective and discounted services for our members.
We advocate for non-insured members to not overpay.
We guide insured members based on their policies to pay least co-pays/deductibles. Assist with Insurance Plan and explore alternate options for benefits and cost savings.
  • Monthly check-ins via phone or video call
  • Access to educational resources on geriatric care and wellness
  • Referral assistance to other healthcare professionals and/or services as needed
  • Priority scheduling for in-home visits
  • Monthly medication management

Standard Membership:

Annual Fee

(12 hours of timed care coordination included)

Services Include:

  • Everything in Basic Membership, plus:
  • Every other month assessment and personalized care plan development
  • 6 In-home or Tele-visits by a care manager for health monitoring and medication management
  • Every Quarter – Coordination of appointments with healthcare providers
  • Emergency support available until 7pm
  • Detailed Care Management Report at end of each month

Premium Membership:

Annual Fee

$2,995

(25 hours of documented and timed care coordination plus additional documented hours are $175/hour)

Services Include:

  • Everything in Standard Membership, plus:
  • Every month assessment and personalized care plan development with end of week log and tasks for upcoming week
  • Weekly in-home visits by a care manager
  • One-hour weekly review of all assistance with activities of daily living (ADLs) such as meal preparation, grocery shopping, and transportation to appointments by coordinating / overseeing existing caregiver support and respite services personnel
  • Advanced Medical Provider Care Coordination for complex medical needs (billed in 15 min increments)
  • Update end of month coordination of medical records to provider(s) and family.

Additional Services (Available to All Memberships):

  • Caregiver Training Workshops: $50 per session
  • Specialized Caregiver Support Groups: $25 per session
  • Geriatric Caregiver Consultation: $100 per hour
  • Registered Nurse Medication Management: $175 per hour

Payment Options:

Membership Renewal:

  • Memberships automatically renew annually unless canceled by the member or the company.
  • Members will receive a reminder one month prior to renewal date.

Terms & Conditions:

  • Membership fees are non-refundable.
  • Services are subject to availability and may vary based on location.
  • Changes to membership tier can be made upon request with 30 days’ notice.
  • The organization reserves the right to modify services and fees with reasonable notice to members.

This membership plan offers flexibility and comprehensive care options for ill or elderly individuals and their families, ensuring they receive the support they need to thrive while living with a diagnosis and in their later years.

Membership Cost

H.O.P.E. Concierge & Access Membership is an annual program. 
Its membership includes access to 12 months of service(s) from date of activation.

Who benefits from this annual membership?
  • Everyone who may want to have access to a resource who can provide them access to quality healthcare providers.
  • Anyone who may have a healthcare issue, question or concern and needs access to a resource, who can guide them to whatever they may need.
  • An insured person who may have a medical need and needs to make best use of their policy’s coverage, especially if they have limited coverage. Even with the best of coverage, members should have a strong advocate who can guide them on how best to use their benefits.
  • An insured person who needs hand-holding once they have been diagnosed with a serious illness or looking for prevention/services.
  • A non-insured person who needs strong advocates to access to cost-effective treatments and services.  
What types of services do members receive? 

Persons have many various needs. However, most members seek our team of Patient Advocates and Navigators for:

  • A team of Patient Advocates and Navigators assist members with:
  • Options to quality healthcare providers for each case, whether prevention and/or treatment(s) to insured and non-insured.
  • We access most cost-effective and discounts to services for our members.
We advocate for non-insured members to not overpay.
We guide insured members based on their policies to pay least co-pays/deductibles.
  • Expedite scheduling of appointments for patients-virtual, telephone and in-person.
  • Coordination of medical records to provider(s).
  • Communication with medical team and/or family. We work as a team with all involved in care of patient’s case.
  • We are member’s liaison in accessing health insurance coverage details.
  • Coordination of ancillary services for members such as diagnostics, air ambulance services and other support services.
  • Provide member traveling from elsewhere with logistics information including discounted lodging, transportation, etc. Please note that some of our providers provide complimentary transportation, a driver, for our members.
  • Assist member with prevention service(s) including comprehensive physical exams, mental health and wellness support services with quality wellness providers.
  • In certain cases, a patient navigator to accompany member to appointments. 
  • For most cancer cases, member has access to complimentary/free/no charge virtual second opinions.
Our network of oncology providers in Miami/South Florida will review each member’s case and provide their opinion for treatment(s).
  • For cases requiring other specialties, outside of cancer, we facilitate with specialists and second opinions at discounted cost.
  • Discounted Executive / Medical Physicals in South Florida.
